This paper evaluates the implementation and impact of the Primary Health Care Transition Fund (PHCTF) project, an initiative of the Community Health and Social Services Network (CHSSN) that is funded by the federal government from the Primary Health Care Access Fund. The PHCTF project is a grant to health and social service institutions that is intended to address a real gap in the provision of services to English-speaking people. The evaluation highlights challenges encountered and shows that the health and social services institutions that received funding consider that the PHCTF project meets a real need for English-speaking people. The funding granted allows for the design and implementation of projects to adapt their service offer to English-speaking people.<\/p>\n
